What Is Trichomoniasis?

Trichomoniasis, often referred to as “trich”, is one of the most common sexually transmitted diseases worldwide, affecting both women and men although, women are at greater risk to contract this disease. Women, usually in the age group of 16 and 35, are most susceptible. The causative organism of Trichomoniasis is a single celled protozoan parasite known as trichomonas vaginalis.

What is this Condition?

Respiratory alkalosis is a condition marked by a decrease in the partial pressure of carbon dioxide of less than 35 millimeters of mercury, which is due to alveolar hyperventilation. Uncomplicated respiratory alkalosis leads to a decrease in hydrogen ion concentration, which causes elevated blood pH. Hypocapnia (decreased carbon dioxide in the blood) occurs when the elimination of carbon dioxide by the lungs exceeds the production of carbon dioxide in the cells.
What Causes it?

Causes of respiratory alkalosis fall into two categories:

pulmonary: pneumonia, interstitial lung disease, pulmonary blood vessel disease, and acute asthma

nonpulmonary: anxiety, fever, aspirin toxicity, metabolic acidosis, central nervous system disease (inflammation or tumor), sepsis, liver failure, and pregnancy.
What are its Symptoms?

The cardinal sign of respiratory alkalosis is deep, rapid breathing, possibly more than 40 breaths per minute. This hyperventilation usually leads to light-headedness or dizziness, agitation, numbness and tingling around the mouth, wrist and foot spasms, twitching, and weakness. Severe respiratory alkalosis may cause irregular heartbeats, seizures, or both.
How is it Diagnosed?

This condition is diagnosed by arterial blood gas analysis, which measures the level of oxygen, carbon dioxide, and other gases in the blood.
How is it Treated?

Treatment is designed to eradicate the underlying condition, for example, removal of ingested toxins, treatment of fever, and treatment of central nervous system disease. In severe respiratory alkalosis, the person may be instructed to breathe into a paper bag, which helps relieve acute anxiety and increases carbon dioxide levels.
How is it diagnosed?

Typical clinical features with appropriate lab data and X-ray findings suggest sarcoidosis. A positive Kveim-Siltzbach skin test supports the diagnosis. In this test, the person receives an injection of an antigen prepared from spleen or lymph node tissue from people with sarcoidosis. If the person has active sarcoidosis, granuloma develops at the injection site in 2 to 6 weeks. This reaction is considered positive when removal and analysis of skin tissue at the injection site shows discrete epithelioid cell granuloma.

Other relevant diagnostic tests include:

chest X-ray

removal and analysis of lymph node, skin, or lung tissue

pulmonary function tests

blood tests and arterial blood gas studies.

A negative tuberculin skin test, fungal serologies, and sputum cultures for mycobacteria and fungi, as well as negative biopsy cultures, help rule out infection.

The food you eat is propelled through your digestive tract by muscular contractions that for the most part are automatic. The process of digestion changes the components of food into a form that eventually will be absorbed into your bloodstream. After the nutrients are absorbed, your digestive tract eliminates unwanted material.

Digestion begins when you chew your food. The food is broken into smaller pieces by your teeth and, at the same time, is mixed with saliva secreted by your salivary glands. Your saliva contains an enzyme called ptyalin that begins to change starches into sugars.

Chewing reduces the food to a mushy consistency. When you swallow, the food is propelled into the back part of your throat, past the opening of the voice box, and into the upper part of your esophagus. Food is prevented from entering your larynx by a flap of soft tissue that closes as food passes into the esophagus. When the epiglottis fails to close completely, a minor coughing fit may result. Such an episode is said to be the result of the food having gone down the wrong tube.

The walls of your stomach consist of various layers of powerful muscles. These muscles serve an important mechanical function as they cause the stomach to churn, breaking the food into smaller and smaller pieces. In addition, gastric juices manufactured by the glands that line your stomach mix with the food particles. These juices contain pepsin, a digestive enzyme that begins to break down proteins in the mixture, and hydrochloric acid, which creates the proper environment for pepsin to work.

Although it serves a useful function, your stomach is not vital to the breakdown of food and its subsequent absorption. Only small amounts of such food as alcohol, simple sugars, and some medications are actually absorbed in the stomach.

There is a delicate balance in your stomach between the acid produced by its glands and the resistance of your stomach lining to that acid. If this balance is upset, the result may be damage to the stomach lining, such as peptic ulcer or gastritis.

Food leaves your stomach in two phases. The upper portion of your stomach contracts first, pushing the more liquid material into your small intestine. The more solid food leaves later, primarily by the action of the muscles in the lower part of your stomach. The partially processed food, chyme, that travels through the pyloric canal into the first portion of your small intestine, duodenum.

By understanding the workings of the esophagus and its control valve, the LES, you can make better decisions on how to deal with any stomach disturbances or with acid reflux.

The esophagus is a muscular channel or tube that runs from the back of the throat to the upper part of your stomach. This channel surface is covered with smooth delicate cells called epithelial cells The muscles along the esophagus start contracting as food enters the esophagus and propels the food downward toward your stomach. These contractions are called peristalsis. It takes about 5 10 seconds for food that you swallow to travel through the esophagus and reach your stomach.

At the end of the esophagus is a closed valve call Lower Esophageal Sphincter or LES. This valve or LES relaxes when food reaches it and opens to let food into your stomach. Once food passes through, it closes tightly so that food cannot travel back up or reflux. It is important for this valve to open and close only to let food flow in one direction, into the stomach.

When you have acid reflux, food and acid from the stomach flows back up into the esophagus causing a burning sensation. When acid reflux or heartburn occurs a few times, this is not a serious issue. But, when it occurs over a long period, then it is possible that an esophageal ulcer or cancer can develop.

The closing pressure of the LES value at its resting point is about 20 mm of mercury. When you eat food a signal is sent to the LES valve to open and allow food swallowed to pass into the stomach. As the stomach contracts to process new food, the LES closes to protect the esophagus from harsh stomach acids.

Every once in a while the LES valve might have a decrease closing pressure. The LES valve relaxes beyond normal and the pressure from the stomach gases are enough to open it. When this happens, food and stomach acid can reflux back into the esophagus. This abnormal momentary decrease in LES pressure is called Transicent LES Relaxations or TLESRs. The amount of times TLESRs happen varies from person to person and researchers are not sure why this happens. Those people who have more frequent TLESRs are more susceptible to acid reflux or heartburn.

It seems to me that these TLESRs can be an interruption or weakening of your defense mechanism or immune system. It is these systems that are responsible for protecting you from disruptions in your body functions. But when physical or emotional forces place a load on your defense and immune systems, not enough energy is available to keep valves shut or valves open as required for normal body functioning.

The simple opening and closing of the LES value is required for good health. Eating excess food or mixing many foods at one meal can increase the pressure in the stomach forcing the LES to open. The rheumatologist usually starts testing for the symptoms with what is called the tender-point test. This is not to be confused with the trigger-point test used to check for Myofascial Pain Syndrome.

In some cases, physicians may suspect fibromyalgia to be hypochondria or depression. A test for Substance P would clarify the diagnosis. Substance P is a neurotransmitter present in higher quantities in those with fibromyalgia syndrome. The more Substance P that is present, the lower pain thresholds appear to be. Even the smallest injury may produce enough pain to dominate the thinking of a person suffering from it.

This may partially explain why those with this condition have a hard time getting good sleep. Many do not reach stage four sleep as often as they need to achieve optimal health. This compounds the pain problem, as injury repair takes place at greater rates during and after good sleep.

Another problem associated with fibromyalgia symptoms is what we often call “brainfog.” A lot of the trouble thinking is probably related to the lack of high-quality sleep. Additionally, the brain is overloaded with messages of pain from the nerves, leaving less processing capacity for normal daily function.

While fibromyalgia is fortunately not a deadly issue, many people do report that it gets worse over time. These people must find ways to cope with the pain. Many turn to support organizations such as fibrofoglights.org and various other Internet support groups. Also, there are medications that your doctor can prescribe that help many people to deal with fibromyalgia syndrome. This type of treatment will have to do until a cure is found.

Various Types of Epilepsy

It is very essential for everybody to be aware of a detailed account of the attack of major (or grand mal or generalized), focal or Jacksonian and petit mal epilepsies. At times, tests like electroencephalogram (EEG) and / or computed tomographic (CT) scanning may be normal in a case of epilepsy, and, therefore, it is only the evidence of the eyewitness, who is in a position to report the details to the attending physician, when the patient is brol1ght to the hospital. As an emergency case, a clinical diagnosis of epilepsy can be made on the spot. Moreover, such cases/emergencies need immediate treatment, and the tests, even if they are conducted, take their own time.

In major (or grand mal or generalized) epilepsy, after momentarily experiencing a peculiar sensation or feeling, i.e. one of the types of auras, described earlier (the aura may even be absent), the patient loses consciousness instantaneously, and he collapses all at once. And if unluckily, the place is odd, e.g., a patient is coming down the stairs or standing near a fire or on a hill top, he may injure himself badly.

Following the fall, all the muscles of the body, i.e. all the muscles of the limbs, abdomen, chest and even face become rigid/ stiff. Due to the spasm of the trunk/respiratory muscles, breathing almost stops, and as the muscles of the face are also involved, the mouth gets firmly closed, and the teeth get clenched. The patient then experiences acute agony, and is close to death. But within a few seconds, all the muscles relax, breathing is regained, and the patient starts getting convulsions of the whole body, which usually continue for about 1-2 minutes, and thereafter, the whole body gets relaxed or becomes absolutely limp.

During such convulsions, the tongue moves repeatedly inside the mouth, and together with the repeated movements of the jaws, the saliva in the mouth gets stirred, and frothing occurs. The tongue may also be bitten when it comes in-between the teeth, and sometimes bleeding may also occur, causing a blood-stained froth. The patient may pass urine (and rarely faces) during this phase of convulsions. It should be remembered that tongue-bite, frothing, passing of urine are important clinical features of an attack of epilepsy, which must be noticed carefully by the eyewitness, for the information of doctor.

The patient may sleep for hours following an attack. However, he may be awakened after a reasonable time when he may complain of a marked headache and pain in all the muscles. When awake, he may have no recollection of his severe fall or convulsions. At the most, he may remember the aura, i.e. transitory symptoms that he may have experienced before the onset of the attack of epilepsy. However, this information about the aura has a great significance in clinical diagnosis, and, therefore, the patient must be coaxed again and again to elicit the required information.

The patient may even get the attack during sleep, and in such cases, tongue-bite or passing of urine in bed should help in the diagnosis. The patient may even die due to suffocation if he happens to turn over during this nocturnal attack of epilepsy, with his face buried in the pillow.

Attacks of grand mal epilepsy may occur many times a day. However, the occurrence of attacks of epilepsy are quite variable to the extent that the patient may not have any further attack during his/her whole life. And hence the value of mass awareness about the clinical diagnosis of epilepsy increases because the treatment cannot be started when a single/first attack of epilepsy occurs, which remains doubtful due to the lack of evidence. On the other hand, the patient may get one attack after the other without regaining consciousness in between the attacks, called status epilepticus, which could be even fatal, if immediate medical aid is not provided.

At times, there is a transitory jerky movement of the limb / s, so that in case both the lower limbs are involved and the patient is standing, he may fall down all at once. This state is called myoclonic epilepsy. Sometimes the small muscles of face, eyes etc. may also be involved.

In petit mal epilepsy, which occurs often in children, there is an extremely transitory attack of unconsciousness. The child, if standing, does not even fall down, and if he/she is doing his/her home work, the child continues with the work as if he/she has had no interruption. The attack of epilepsy may occur repeatedly even 15, 20, 30 times a day. When the attacks are limited to 1 to 2 per day, or on alternate days or even less, the child may remain unaware of such an attack. And even if he/she brings it to the notice of the parents, it may be ignored altogether.

However, whenever a child brings the attack to the notice of his parents, they. must take it very seriously, and tests like EEG, CT scan etc. should not be delayed. Therefore, the knowledge of symptoms of petit mal epilepsy is most important for parents, and even for teachers in schools, so that the disease may not remain undetected especially in early and occasional cases. However, in rare cases, the child may even collapse during unconsciousness. This is called an akinetic attack/ epilepsy, and thus, in such cases, there is usually no difficulty in diagnosis.

The attacks of petit mal epilepsy may disappear altogether on attaining maturity, although the treatment will have to be commenced as soon as the diagnosis is confirmed.

It is urgent that all persons, particularly all members/ friends/colleagues concerned with the case must keep in mind all the relevant signs and symptoms whenever the patient is to report to the physician for diagnosis.
